I used to add a custom attachment field (show/hide) based on the field value in the older portal using JQuery.
Now after updating to the latest portal, this has stopped working. Is there a way to add multiple attachment fields in the Service Request form as shown in the below image?

Thanks for the response.
I used as usual tag in the Page Layout file as did in the older version.
Did not use any liquid syntax. Is there any samples which I can refer?
