I need to render the app in the sidebar of new tickets page

The client wants me to create a form and add it in the new ticket page on the sidebar as mentiened in the picture, I need to know the name of the location plz

It’s the New Ticket Page, and the placeholder name is new_ticket_requester_info.

The app is displayed on the CONTACT DETAILS tab of the New ticket page.
