onConversation Create and onConversation update Events trigger issue

Hi Team,

We have created a custom app, in which app perform certain action based “on conversation create” event in the freshdesk.

But the event did not get triggered for few replies to a conversation by the agent.

To confirm this issue, we checked by replying to the conversation of a ticket and we have seen that, for few agent replies only "onConversation update " event getting triggered and for some replies both “onconversation create” and “onconversation update” events getting triggered at same time. And for some replies only “onConversation Create” event got triggered.
